It is an unlikely situation, but imagine this:
There has been an unfortunate accident in a battle where an elderly Medicine cat was killed by a rival warrior. The Medicine cat had taken on an apprentice in the past, but that apprentice sadly died from sickness(ironic). There were no kits in the nursery to be apprenticed after that, and the warrior apprentices were only a couple days away from their Warrior ceremony, making getting a new Medicine cat apprentice impossible. Therefore, the MC died without passing on the medicinal knowledge. The other members of the clan know very little about herbs and/or their uses.
What happens when a Medicine cat dies without having passed their skills onto an apprentice? How would a new Medicine cat be chosen/trained?
Would the most knowledgeable warrior take over?
It is an interesting question, and having read the books years ago, it kinda slipped my mind. I do, however, remember the "warrior-turned-Medicine-cat", Hawkheart from Bluestar's Prophecy but it was never explained why or how Hawkheart changed rank. Unless I'm missing something, that is. What do you guys think?

Perhaps the leader would go to consult starclan, or maybe they would start to look outside of the clans, much like how Leafstar found a medicine cat for SkyClan...

This has actually happened in the books before! It was briefly discussed in Goosefeather's Curse, stating (taken from the wikia) "Goosekit instantly remembers that Cloudberry was from RiverClan but joined ThunderClan after the Clan’s only medicine cat Ravenwing was killed."

Also in, In To The Wild Yellowfang was chosen because she was a past medicine cat. But I agree that another medicine cat from another clan could train a apprentice to be a medicine cat.

I think another medicine cat from another clan will train someone, I'm pretty sure I have seen that happen in the books. Haven't read one is a while so I'm not too sure.

This is a tricky one, if it were leader and deputy it would be FIGHT TO TEH DEATH, but I'm pretty sure being the best fighter would mean being the best medicine cat xD

^most of those ideas are possible, but I feel like some really harsh leader might be like "oh we don't need med cats", and survive without them, and the weak cats just die.
